Using context to decipher the indecipherable. No matter how good your recording is, there will be moments that are unintelligible. This is where a good transcriptionist steps in to fill in the blanks. This requires extra time, maybe some internet research, and language expertise .
Researching the garbled but knowable. Your interviewee drops the name of the university he studied at in Sweden and the surgery technique he pioneered. I look these up so you don't have to.
Crafting a readable document. Unless you request a verbatim transcript, I create a grammatically-correct, flowing text that is easy to read and useful for cutting and pasting from later.
